Introduction: Cystic renal cell carcinoma (CRCC) is an uncommon special subtype of renal cell carcinoma (RCC) associated with a low nuclear grade and stage and an excellent prognosis regardless of tumor size. Case presentation: A 65-year-old male presented with right flank pain of 2-month duration. Radical nephrectomy was done. Discussion: There has been explanation of the four mechanisms that account for RCC with cystic features: Intrinsic unilocular cystic growth which results from intrinsic growth and comprises approximately 30 per cent of cases of cystic renal cell carcinoma and intrinsic multilocular cystic growth which consists of multiple variable sized, non-communicating, fluid-filled cystic spaces. Tumor can also originate in a preexisting simple renal cyst. Lastly tumor necrosis can result in cyst formation (pseudo cyst) in which cystic space is filled with necrotic debris and hemorrhage. Conclusion and recommendation: It is difficult to differentiate Cystic renal cell carcinoma from benign renal cyst and renal cell carcinoma. It is important to do evaluation and continuous follow-up on the management of simple renal cysts.
