Heterotopic pregnancy – the simultaneous occurrence of an intrauterine pregnancy (IUP) and an extrauterine (most commonly tubal) pregnancy – is a rare but increasingly recognised diagnosis that poses a significant diagnostic and management challenge. We present the case of a primigravida woman at approximately seven weeks’ gestation who presented with abdominal pain and vaginal bleeding. Transvaginal ultrasound demonstrated a viable intrauterine pregnancy with peri-gestational bleeding alongside a separate left adnexal mass suspicious of a coexisting tubal ectopic pregnancy. Following appropriate counselling and consent, the patient underwent diagnostic laparoscopy, which confirmed a ruptured left tubal pregnancy with approximately 250 ml of haemoperitoneum. A left salpingectomy was performed, with careful operative technique employed throughout to safeguard the ongoing intrauterine pregnancy. This case illustrates the importance of maintaining a high index of suspicion for heterotopic pregnancy, the principles of safe laparoscopic surgery in early pregnancy, and the value of thorough pre-operative counselling when diagnostic certainty cannot be guaranteed.
